What your fitness certificate looks like
Every certificate is issued by a registered doctor after a genuine online assessment. Below is an illustrative sample - your details, purpose, and validity dates appear on the real document.
- The issuing doctor’s credentials and registration number
- Your details and the outcome of the health assessment
- The purpose and how long the certificate stays valid
- Delivered as a same-day PDF with a unique reference code
- Suitable for the gym, sports, travel, work or study

Quick heads up: this page covers non-competitive activity. Gym membership, personal training, fit-to-fly, return to work, school trips and recreational diving all live here. If you’re after a certificate for organised or competitive sport, martial arts, contact sports or a return-to-play assessment after injury, that’s a different document. Head to our sports fitness certificate page instead.
What Is a Fitness Certificate?
A fitness certificate is a document issued by a registered doctor confirming you’re medically fit to take part in a stated activity. It’s based on a health questionnaire and clinical review, and it stays valid for 12 months from the date of issue. You’ll use it for things like gym sign-up, fit-to-fly clearance, or getting back to work after illness.
This document isn’t the same as a few other papers people confuse it with. Let’s break down the difference.
Fitness Certificate vs Sick Note vs Sports Fitness Certificate
A fitness certificate confirms you’re able to do something. A sick note does the opposite. It certifies that you’re currently unable to work or carry out normal activities because of illness or injury. Same doctor-issued format, completely reversed purpose.
A general medical certificate is broader still. It confirms your overall health status at a point in time, without necessarily saying anything about your capacity for a specific activity. A fitness certificate is narrower and more useful for a gatekeeper. It answers one question only: can you do this particular thing?
Then there’s the sports fitness certificate, which covers organised and competitive sport, martial arts, contact sports and return-to-play situations. That’s a separate product with its own clinical questions. If your reason for needing a certificate involves a league, a club, a competition or contact sport, that page is the right one, not this one.
| Document | Confirms | Typical Use |
|---|---|---|
| Fitness certificate (this page) | You’re medically fit for a stated non-competitive activity | Gym membership, fit to fly, return to work, school trips, diving |
| Sick note | You’re currently unable to work or carry out normal activities | Sickness absence from work or study |
| Sports fitness certificate | You’re fit for organised or competitive sport | Sports clubs, martial arts, contact sports, competitions |
How It Works
Getting a fitness certificate online follows three steps, and most people are done inside half an hour.
Complete a Health Questionnaire
You’ll state the purpose of the certificate and answer questions about your current health, medical history and any relevant conditions.
A Doctor Reviews Your Answers
If anything needs clarifying, the doctor may request a short video consultation before making a decision.
You Receive Your PDF
Once approved, your fitness certificate online arrives by email as a signed PDF with a unique verification reference, typically within 30 minutes.
Simple. No clinic visit, no waiting room, no appointment three weeks out.
What You Can Use It For
This certificate is built for non-competitive, everyday situations where someone needs written confirmation that you’re fit to take part. Here’s what it covers:
Gym Membership and Personal Training
Many gyms and personal trainers ask new members for medical clearance before starting a program.
Fit to Fly
Confirmation that you’re medically fit to travel by air, for standard situations.
Return to Work
Confirmation you’re fit to resume duties after illness or injury.
School Trips and Residential Activities
Schools often require fitness confirmation before a student joins a trip, outdoor activity week, or residential program.
Recreational Swimming and Scuba
General fitness confirmation for recreational water activity.
Insurance and General Activity Clearance
Some insurers and activity providers ask for a baseline fitness confirmation before cover starts.
Worth noting: a fitness certificate confirms general medical fitness for these activities, but it doesn’t override any specific process an organisation runs on its own. More on that in the next section, because it’s the part most services skip over.
| Purpose | What the certificate shows |
|---|---|
| Gym membership | General medical fitness to begin low-to-moderate intensity exercise |
| Fit to fly | General medical fitness to travel by air in standard circumstances |
| Return to work | General medical fitness to resume normal duties |
| School trips | General medical fitness to take part in a stated activity or trip |
| Recreational diving | General medical fitness for recreational, non-competitive water activity |
| Insurance clearance | Baseline confirmation of medical fitness at time of assessment |
Who Decides Whether It’s Accepted
Here’s what most people don’t realize: the doctor who issues your fitness certificate isn’t the one who decides whether an airline, employer or gym accepts it. Each purpose has its own decision-maker, and some of them run a completely separate process you’ll need to know about upfront.
| Purpose | Who decides | What they typically require |
|---|---|---|
| Gym membership | The gym operator | Accepted by gym operators that accept certificates from registered doctors. In Italy, national law requires a certificato non agonistico with a baseline ECG for gym membership, so a questionnaire-based certificate alone won’t satisfy that requirement there. |
| Fit to fly | The airline’s medical department | Airlines run their own medical clearance process and often require their own MEDIF form completed by a treating physician, or review by an aviation medical examiner. The issuing doctor’s certificate supports your case, it doesn’t replace the airline’s own decision. |
| Return to work | Your employer, subject to national rules | In the Netherlands, employers may not request a GP certificate or diagnosis for sickness absence. Assessment runs through the bedrijfsarts (company doctor) via the Arbodienst, and the employer only receives functional advice on what work you can and can’t do. In Germany, occupational fitness runs through the company physician rather than a general practitioner. |
| School activities | The school or institution | The institution reviews the certificate against its own trip or activity policy. |
| Scuba diving | The training agency | Most diving training agencies require their own medical questionnaire or examination form, signed after a physical assessment, rather than a general fitness certificate. |
| Insurance | The insurer | The insurer reviews the certificate alongside its own claim documentation and underwriting requirements. |
Bottom line: get your fitness certificate online, but always check what the receiving organisation actually needs before you rely on it for something regulated, like employment or air travel.
What the Doctor Assesses
Your doctor works through a structured health questionnaire covering both general health and anything specific to your stated purpose.
General Health Screening
- Current medical conditions
- Recent illness, injury or surgery
- Current medications
- Family history of cardiac conditions
- Symptoms during physical exertion, such as breathlessness or chest pain
Activity-Specific Considerations
For gym membership and fit-to-fly requests, the doctor looks closely at cardiovascular history and any recent hospital treatment. For return-to-work certificates, the focus shifts to the nature of the illness or injury and whether it affects the specific duties you’ll be resuming.
For recreational diving, the doctor asks about your history of ear and sinus problems, seizures, and cardiac symptoms as part of the questionnaire. This is history-taking, not a physical exam. Most diving training agencies will still want their own medical form completed, often after an in-person assessment, before you dive.
| Assessment area | Method |
|---|---|
| Current conditions and medications | Questionnaire |
| Cardiac history | Questionnaire, family and personal history |
| Exertional symptoms | Questionnaire |
| Ear, sinus and seizure history (diving) | Questionnaire only, not physical examination |
| Physical examination, ECG or lung function testing | Not included |
Is an Online Fitness Certificate Valid?
Yes. A fitness certificate issued by a doctor registered in your country, after a proper clinical review, is a valid medical document. Three things make it valid: the doctor is registered to practise, their credentials and registration number appear on the document, and the doctor is licensed to practise where you’re physically located when assessed.
Here’s the part worth being honest about: validity isn’t the same as acceptance. Your certificate can be entirely valid as a medical document while still not satisfying a specific organisation’s own process. Where a statutory route exists, like occupational health assessment in the Netherlands or Germany, or where a body runs its own accreditation scheme, like an airline or a diving agency, that process governs. Check the table in the previous section for exactly who decides in your situation.
| Validity factor | What it means |
|---|---|
| Doctor registration | The issuing doctor holds a current license with their national medical regulator |
| Credentials on document | Doctor’s name, registration number and country appear on the certificate itself |
| Location-based licensing | The doctor is licensed to practise in the country where you’re physically located during assessment |
| Validity vs acceptance | A valid certificate can still be insufficient for a specific statutory or organisational process |
What’s on Your Certificate
Every fitness certificate online includes the same set of fields, so whoever receives it can check exactly who issued it and on what basis.
- Your personal details
- Date of assessment
- Stated purpose of the certificate
- The doctor’s findings and conclusion
- Doctor’s name, registration number and country of registration
- Digital signature from the issuing doctor
- Unique verification reference
- Validity period
The digital signature confirms the document was signed by the named doctor. It doesn’t carry a single universal legal status. Signature law differs by country, so if you need to know how a specific receiving organisation treats digitally signed documents, that’s worth checking with them directly.
| Field | Purpose |
|---|---|
| Doctor’s registration number | Lets the recipient verify the doctor’s license with the relevant regulator |
| Verification reference | Unique code allowing an institution to confirm the certificate is genuine |
| Validity period | Confirms the 12-month window during which the certificate applies |
| Stated purpose | Ties the certificate to the specific activity it was assessed for |

What We Cannot Certify
Real talk: no online questionnaire-based service can replace every regulated medical process, and pretending otherwise would set you up to fail somewhere important. Here’s exactly where this fitness certificate doesn’t apply.
- Statutory occupational health assessment. In the Netherlands, pre-employment medical examinations are restricted by law to roles with defined medical requirements, such as divers, firefighters, pilots and professional soldiers, and must be carried out by an authorised physician. For most jobs, this kind of examination isn’t permitted at all. In Germany, occupational fitness runs through the company physician, not a general practitioner.
- Airline-specific clearance. Where an airline requires its own MEDIF form or assessment by an aviation medical examiner, our certificate supports that process but doesn’t replace it.
- Italian gym membership. Italian law requires a certificato non agonistico, including a baseline ECG, for gym membership and non-competitive sport. A questionnaire-based certificate can’t satisfy this on its own.
- Diving medicals requiring examination. Where your training agency requires a physical examination and its own signed medical form, our certificate doesn’t substitute for that.
- Competitive sport. For organised or competitive sport, martial arts, contact sports or return-to-play situations, use our sports fitness certificate instead.
- Conditions needing physical examination, imaging or lab testing. Some health questions can’t be answered through a questionnaire alone.
- Cases with insufficient clinical basis. If your answers don’t give the doctor enough to make a safe decision, the certificate won’t be issued.
| Situation | Why this certificate doesn’t apply |
|---|---|
| NL/DE pre-employment or return-to-work | Reserved by law to authorised physicians or the company doctor |
| Airline-required MEDIF form | Airlines run their own medical clearance process |
| Italian gym membership | Requires a certificato non agonistico with baseline ECG |
| Diving medical requiring examination | Training agencies often require their own signed form after physical assessment |
| Competitive sport | Covered by our sports fitness certificate, not this document |
Our Doctors
Every fitness certificate online is reviewed and signed by a doctor registered with a recognized medical regulator in their home country.
- Dr. Sinéad Whelan, IMC registration #114892, Ireland
- Dr. Lukas Brandt, ÄK registered, Germany
- Dr. Camille Dubois, RPPS registered, France
- Dr. Anna Kowalska, NIL registered, Occupational Health, Poland
Dr. Kowalska’s Occupational Health registration is worth flagging here specifically, since it’s directly relevant to return-to-work questions, even though the statutory occupational health route in countries like the Netherlands and Germany still runs through their own designated practitioners for regulated cases.

| Dr. Lukas Brandt | ÄK | Germany |
| Dr. Camille Dubois | RPPS | France |

Verify a Certificate
Every fitness certificate carries a unique verification reference. If you’re an employer, gym, school or airline checking a certificate someone has given you, that reference lets you confirm it was genuinely issued by one of our registered doctors, and that it hasn’t expired or been revoked.
Simply enter the reference number on our verification page and you’ll get an instant confirmation of status, issue date and validity.
| What verification confirms | Detail shown |
|---|---|
| Certificate authenticity | Confirms the document was issued by a registered doctor on our platform |
| Issue date and validity | Shows whether the certificate is still within its 12-month validity period |
| Current status | Confirms the certificate hasn’t been revoked or superseded |
Your Privacy
Your health information is handled under GDPR, encrypted in transit and at rest. The verification reference on your certificate exposes only the certificate’s status and validity, nothing about the health information behind it.
You can request deletion of your data at any time, subject to the record-keeping periods doctors are required to follow under their own regulatory obligations.

Frequently Asked Questions
| Question | Short answer |
|---|---|
| Pre-existing conditions | Disclosed on the questionnaire, reviewed case by case |
| Multiple purposes | One certificate per stated purpose |
| Physical examination | Not required, questionnaire-based |
| Fit to fly | Yes for standard situations, some airlines need their own forms |
| Doctor declines | Full refund issued |
What if I have a pre-existing condition?
You’ll disclose it on the health questionnaire, and the doctor reviews it as part of your assessment. Many pre-existing conditions don’t prevent a certificate being issued, but the doctor needs the full picture to make a safe decision for your stated purpose.
Can I use one certificate for multiple purposes?
Your certificate states a specific purpose, and it’s assessed against that purpose. If you need a certificate for a second, different purpose, like fit-to-fly and gym membership, you’ll need a separate assessment for each so the doctor’s findings match what’s stated.
Do I need a physical examination?
No. Assessment is based on a health questionnaire, with a video consultation if the doctor requests one to clarify your answers. There’s no in-person clinic visit required to get your fitness certificate online.
Can I use this for fit to fly?
Yes, for standard fit-to-fly situations. Keep in mind some airlines require their own MEDIF form completed by a treating physician, or assessment by an aviation medical examiner. Our certificate supports your case, but the airline’s medical department makes the final call.
What happens if the doctor declines to issue my certificate?
You’ll get a full refund. Doctors decline when the questionnaire doesn’t give enough clinical basis for a safe decision, or when your situation needs an in-person examination the online process can’t provide.
Is a fitness certificate the same as a doctor’s note?
Not quite. A doctor’s note or sick note usually certifies you’re unable to work or take part in an activity. A fitness certificate does the opposite, confirming you’re medically fit for a stated activity.
How long does it take to get my fitness certificate online?
Typically within 30 minutes of completing your questionnaire, assuming the doctor doesn’t need a follow-up video consultation. You’ll receive a signed PDF by email with your unique verification reference included.
Will my gym accept this certificate?
Many gym operators accept certificates issued by registered doctors. One exception worth knowing: Italy requires a certificato non agonistico with a baseline ECG for gym membership, which this questionnaire-based certificate can’t satisfy on its own.
Can I use this certificate for return to work in the Netherlands?
Dutch employers aren’t permitted to request a GP certificate or diagnosis for sickness absence. Assessment runs through the bedrijfsarts via the Arbodienst, and the employer only receives functional advice. Check with your employer before relying on our certificate for this specific situation.
Is my fitness certificate valid in every country you cover?
Your certificate is a valid medical document wherever the issuing doctor is licensed to practise, across our 40+ covered countries in Europe and the EEA. Validity is separate from acceptance though, so always check what the receiving organisation in your specific country actually requires.
