What Is a Sick Leave Certificate?

A sick leave certificate is a document issued by a licensed doctor confirming that a patient is unfit for work, study or other duties because of illness, injury or a mental health condition. It states the dates the person is unfit, the doctor's name and registration details, and enough clinical context for an employer or institution to accept the absence.

You'll usually need one when your absence runs past what your employer allows through self-certification, or when a school, university or booking provider asks for written proof.

Sick Leave Certificate vs Fit Note vs Doctor's Note

These three terms get used loosely, and that causes confusion. Here's how they actually differ.

Sick Leave Certificate vs Fit Note vs Doctor's Note
TermWhat it meansWhere it's used
Sick leave certificateGeneral term for a doctor's document certifying unfitness for work or study, used across most of Europe.Employers, schools, universities, travel providers
Fit noteThe current UK term for the same type of document, replacing the older "sick note" wording used before 2010.UK employers, from day 8 of absence
Doctor's noteInformal, everyday phrase for any note a doctor writes, including for shorter absences that don't need a formal certificate.Casual use, not a legal or standardised term

When You Need a Sick Leave Certificate

You need a sick leave certificate any time an employer, school, or service provider requires written medical proof of your inability to work, study or travel. Exactly when that kicks in depends on your country and your situation.

Who Needs One

  • Employees who've exceeded their employer's self-certification period, or whose contract requires certification from day one
  • Students who need to explain a missed exam, deadline or placement
  • Freelancers and professionals who need documentation for clients, insurers, or contractual obligations
  • Travellerswho need proof of illness for visa applications, flight cancellations or rebooking claims
Who it's for

Who uses a sick leave certificate?

Employees

Cover a sick day for your employer without a trip to the GP.

Students

Notes for university, exams or placements, accepted by most institutions.

Freelancers

Documentation for clients or income-protection claims when you can't work.

Travellers

Fit-to-fly letters and cancellation notes for airlines and insurers.

Carers

Certificates for carer's leave when someone who depends on you is unwell.

Which Conditions Qualify

  • Short-term illness, such as flu, infections or migraines
  • Long-term or chronic conditions requiring ongoing management
  • Mental health conditions, including stress, anxiety and depression
  • Injuries that limit your ability to work or attend in person

There's no minimum number of sick days required before you can request a certificate. If you need one, you can ask.

Traveller with a cancelled tripCertificate submitted to the airline, insurer or visa authority

What a Valid Sick Leave Certificate Contains

A valid sick leave certificate includes the patient's name and date of birth, the consultation and issue dates, a diagnosis summary, the specific dates the patient is unfit for work, and the doctor's name, registration number and country of registration, all on official letterhead with a signature and stamp.

These are the elements employers typically require and expect to see, though exact expectations can differ by employer and by country.

Consultation dateThe date the patient was assessed by the doctor

Unfit datesThe specific start and end dates the patient is certified unfit for work or study
Doctor's detailsName, registration number and country of medical registration

Some receiving institutions, particularly abroad, ask for an apostille or legalisation on top of the standard certificate. Check with the receiving party before you submit, since this adds processing time. A certificate should also be recent. One issued weeks before your absence won't reflect your current condition and is unlikely to be accepted.

Is an Online Sick Note Legal?

Yes, an online sick note is legal when it's issued by a licensed physician following a genuine clinical assessment, and when it carries the doctor's credentials and registration number. What varies is how different countries and employers treat it once issued.

The doctor issuing your certificate should be licensed in the country where you're physically located at the time of the consultation. This is the working standard for cross-border telemedicine across Europe.

Ireland is a clear proof point here. Certificates issued by IMC-registered online GPs are valid for both Statutory Sick Pay and Illness Benefit claims, with no distinction made between an online consultation and an in-person one.

Don't assume legal treatment is identical everywhere. It isn't. Requirements differ by country, and it's worth checking your own national rules before relying on any certificate for a formal claim.

Sick Leave Rules by Country

Sick leave certification rules differ sharply across Europe, both in when a certificate is required and what happens if you're late submitting one.

Sick Leave Rules by Country
CountryRule
United KingdomSelf-certification covers the first 7 calendar days of absence, including weekends and bank holidays. A fit note is required from day 8 if the absence continues. Employers cannot insist on a GP note for the first 7 days.
IrelandMedical certification is required from day 1 to claim Statutory Sick Pay, under the Sick Leave Act 2022. Employees are entitled to 5 days of paid sick leave in 2026, at 70% of usual daily earnings, capped at €110 per day, after 13 weeks of continuous service.
GermanyUnder §5 EFZG, a certificate is due by the fourth day of illness exceeding 3 consecutive calendar days, though employers may contractually require one from day 1. The employee must notify the employer on day 1 but doesn't need to disclose the diagnosis.
FranceThe certificate must be sent within 48 hours, under Article R.321-2 of the Code de la Sécurité sociale. It goes to both the employer and CPAM. Late submission can reduce daily allowances, and a repeat within 24 months cuts the payment by 50% for the late period.
NetherlandsEmployers may not ask for a GP certificate at all, nor ask about the diagnosis. Assessment instead goes through the bedrijfsarts (company doctor) via the Arbodienst, who provides only functional work advice.

Rules like these change. Always check both your country's current law and your employer's own policy before relying on a certificate for pay or leave purposes.

What We Cannot Certify

We cannot issue certificates for significantly backdated absences, for cases where the reviewing doctor finds no clinical need, or for conditions that genuinely require a physical examination to assess.

A sick leave certificate also isn't a substitute for a formal occupational health assessment. In Germany, that assessment runs through the Betriebsarzt. In the Netherlands, it's the bedrijfsarts. Neither of those processes can be replaced by an online certificate. We also don't provide emergency care, so anyone with an urgent medical need should contact local emergency services directly.

What We Cannot Certify
LimitationWhy
Significantly backdated certificatesCertification reflects the doctor's assessment at the time of consultation, not retrospectively
No clinical need foundA licensed doctor won't certify unfitness that isn't clinically supported
Occupational health assessmentsGermany and the Netherlands route this through the Betriebsarzt or bedrijfsarts, not an online certificate
Conditions needing physical examinationSome conditions can't be safely assessed remotely
Emergency careNot a substitute for urgent or emergency medical services

How Employers Verify a Certificate

An employer can verify a certificate by emailing the issuing doctor at the address printed on the document, using an official organisational email address, and attaching a copy of the certificate.

Details aren't released to requests that don't meet those conditions. This protects patient confidentiality while still giving employers a genuine way to confirm authenticity.

Frequently Asked Questions

Can I get a sick leave certificate for a mental health condition?

Yes. Mental health conditions, including stress, anxiety and depression, are valid reasons for a sick leave certificate. The doctor reviewing your case assesses your symptoms the same way they would for a physical illness.

Can a sick leave certificate be backdated?

A certificate can reflect a recent onset of symptoms, but significantly backdated certificates aren't issued. The certificate needs to reflect the doctor's genuine assessment at the time of consultation.

What happens if my employer rejects my sick leave certificate?

Your employer can verify the certificate directly with the issuing doctor by emailing the address on the document from an official organisational address. If they still have concerns, check your national rules under Section 6, since acceptance requirements differ by country.

Can I extend my sick leave certificate if I'm still unwell?

Yes, but an extension needs a new assessment. The original certificate can't simply be extended without a fresh review of your condition and current symptoms.

What if I get sick during annual leave?

A sick leave certificate can document illness that occurs during a period of annual leave. Whether that time is reclassified as sick leave instead of annual leave depends on your employer's policy and your country's rules.

Can students use a sick leave certificate for missed exams?

Yes. Students commonly use a sick leave certificate to explain a missed exam, deadline or placement to their institution's exams office or department.

Do shift workers need a different type of certificate?

No, the certificate itself is the same. It states the dates you're unfit for work, and your employer applies that to whatever shift pattern you're contracted to.

Can changes be made to a certificate after it's issued?

No, certificates can't be changed after issue. If your dates or circumstances change, you'll need a new assessment rather than an amendment to the original document.

Is there a minimum number of sick days needed to request a certificate?

No. There's no minimum threshold. You can request a certificate whenever you need documented proof of your unfitness, whatever the length of your absence.

Does a sick leave certificate need to state my exact diagnosis?

Your certificate includes a diagnosis summary along with your unfit dates and the doctor's credentials. This gives your employer or institution enough information to accept the certificate, while your fuller personal health details stay confidential with us.

How is a medical certificate different from a sick leave certificate?

A medical certificate is a broader document that can cover general health matters, while a sick leave certificate specifically certifies unfitness for work or study over defined dates.

When would I need a fitness certificate instead?

You'd need a fitness certificate when you're confirming you're fit to do something, such as starting a job or a physical activity, rather than confirming you're unfit due to illness.

What's a sports fitness certificate used for?

A sports fitness certificate confirms you're medically fit to take part in a specific sport or physical activity, often required by clubs, leagues or event organisers.

Who typically asks for a good health certificate?

Good health certificates are often requested for visa applications, new employment, insurance purposes or immigration processes, where a general statement of health is needed rather than proof of illness.

What is a referral letter used for?

A referral letter is used when your case needs further assessment from a specialist. It's a different document from a sick leave certificate, which certifies unfitness rather than requesting further care.

Can I get more than one type of certificate at the same time?

Yes, depending on your situation you may need more than one document, for example a sick leave certificate for your employer and a referral letter for ongoing specialist care.

Do all these certificates cost the same?

Pricing is specific to each certificate type. Check the relevant certificate page for current pricing rather than assuming they match the sick leave certificate price.

Are these certificates issued by the same doctors?

Yes, all certificates are reviewed and issued by our licensed doctors, with their qualification and country of registration shown on each document.

Which certificate do I need for a missed flight due to illness?

A sick leave certificate can serve this purpose for travellers, since it documents your illness and unfit dates, which airlines and insurers typically require for cancellation or rebooking claims.

How do I know which certificate is right for my situation?

Start with what the requesting party is asking for. If they need proof you're unfit for work or study, a sick leave certificate is the right document. If they need proof you're fit for an activity, choose a fitness or good health certificate instead.
