Which medical certificate do I need? Find the right document in three questions

The right medical certificate depends on who is asking and why. An employer usually wants a sick leave certificate, a club a sports fitness certificate, an airline a fit to fly letter, and a consulate a visa health certificate. Some requests need an exam in person, a named doctor or a test, so check before you order.

Which medical certificate do I need?

Match who is asking and why to the right document in three questions.

Question 1 of 4

Are you very unwell right now?

Answer yes if you have any of these:

  • chest pain or pressure
  • severe difficulty breathing
  • sudden weakness, numbness, facial droop or trouble speaking
  • a seizure, fainting or confusion
  • heavy bleeding that will not stop
  • thoughts of harming yourself
Who is asking?

Answer the questions to see which document usually fits.

How to use this tool

  1. Answer the urgent care question. If any warning sign applies, stop and get help now.
  2. Pick who is asking: employer, school or university, sports club or event, airline or insurer, consulate, or someone else.
  3. Pick why they are asking: an illness absence, returning to work, fitness for an activity, travel, or a visa.
  4. Choose the country where the certificate will be used, and say whether the person asking named a form, doctor, clinic or test.
  5. Read the result. It names the document, links to the tool with the detail, and says whether an online certificate can help.

Why does it matter who is asking?

Because the person asking sets the rules. The same doctor's note can be enough for one employer and useless for a consulate.

An employer checks a sickness absence. A club checks you can take part safely. An airline checks you can fly. A consulate checks a public health rule set by its government. Each one decides which documents it accepts.

That is why the tool starts with the requester, not with the certificate name. Start with who asked, and the right document usually follows.

Schools, colleges and clubs each decide which certificates they accept. Employers can often ask for more than the legal minimum. Airlines and consulates often publish their own forms. So the safest first step is always the same: ask the person who wants the certificate exactly what they need.

What are the main types of medical certificate?

Most requests fall into six types. Four of them we can offer after a doctor reviews your case. Two are planned.

The main types of medical certificate
CertificateWhat it saysWho usually asksCan we help?
Sick leave certificateYou were too unwell to work or study for set datesEmployer, school, universityYes, after a doctor's review
Fitness certificateYou are fit for a named job, course or activityEmployer, course, gym, volunteering roleYes, if the person asking accepts one from any doctor
Sports fitness certificateNo reason found that you cannot take part in a sportClub, gym, race organiserYes, valid 12 months, if no exam in person or test is required
General medical certificateA doctor's statement for a purpose the others do not coverInsurer, organiser, other bodiesYes, if the person asking accepts one from any doctor
Fit to fly certificateYou are well enough to travel by airAirlinePlanned. Join the waitlist
Return to work certificateYou can resume work, often with changesEmployerPlanned. Join the waitlist

Some documents sit outside this list on purpose. A visa health certificate, a driving licence medical and many competitive sports certificates follow rules set by a government or federation. Those usually need a named form, a named doctor or a test. The tool tells you so and points you to the right place.

Routing table: who is asking, why, and what you need

This table shows the route the tool follows for the most common requests. “Check first” means ask the person who asked whether they accept a certificate from any registered doctor after an online consultation.

If the person asking has named a specific form, doctor, clinic or test, the answer changes. Use exactly what they named. An online certificate from us will not replace it.

How the finder decides: the decision flow

The tool follows the same steps every time, in this order. You can follow them by hand too.

  1. Are you very unwell right now? If yes, stop and call 112 or your local emergency number. A certificate can wait.
  2. Is the request for a visa or from a consulate? If yes, the destination country sets the rule. Go to the visa finder. We do not offer a certificate for this today.
  3. Is it for a driving licence? If yes, the licensing authority sets the form and who fills it in. Go to the driving licence page. We do not offer a certificate for this.
  4. Has the person asking named a form, doctor, clinic or test? If yes, use exactly that. An online certificate is not the right route.
  5. Is it a sports club, gym or race? Check your country in the sports tool. If the rule needs a sports medicine specialist, an ECG or another test, book in person. If your club confirms in writing that a note from any doctor is enough, our sports fitness certificate may fit.
  6. Is it an airline? Use the fit to fly checker, or the pregnancy calculator if you are pregnant. Our fit to fly certificate is planned, so join the waitlist.
  7. Is it about going back to work? In the UK you usually need nothing. Elsewhere, ask your employer what it wants. Our return to work certificate is planned.
  8. Is it an employer asking about a sickness absence? Check whether your country lets you self certify first. If a certificate is due, our sick leave certificate may fit.
  9. For everything else, ask the person asking whether they accept a certificate from any registered doctor after an online consultation. If they say yes, our fitness or general medical certificate may fit.

When is an online certificate not the right route?

When the rule needs something a remote consultation cannot give: a physical exam, a test, a named doctor or a local clinic. In those cases, book in person and do not buy an online certificate.

Here are the cases we see most often.

  • Exams in person. Many competitive sports certificates need an ECG, a breathing test, urine or blood tests, or an exercise test. Those need equipment and a visit.
  • Panel doctors and named clinics. For a UK visa, the TB test is done at a clinic approved by the Home Office. Belgium asks for a certificate on its official model from an accredited doctor, or extra authentication otherwise.
  • Sports medicine specialists. In Italy, the competitive sports certificate can only come from a sports medicine specialist after tests. Poland, Luxembourg and Croatia also name specialists or authorised centres for some levels.
  • Driving licence medicals. The licensing authority names the form and who completes it. In the UK, the medical for a lorry or bus licence goes on form D4, filled in by a doctor.
  • Checks after arrival. France, the Netherlands, Luxembourg, Iceland, Norway, Malta, Cyprus, Latvia, Slovakia, Croatia and Romania test or examine some newcomers in the country. A certificate from home does not replace that.
  • National electronic systems. In some countries the sick leave certificate travels through a national system. In France, paper sick leave notices must use the secure official form, and scans or photocopies are not accepted.

In all of these, the tool shows what to book instead and never shows our price.

Do I need a medical certificate for my employer?

Often, but not always on day one. It depends on your country and on how long you are off.

Some countries let you self certify for a few days. The UK allows up to 7 days in a row, counting weekends. Sweden allows 7 calendar days, and Germany 3 calendar days. Others, such as Italy and Croatia, expect a doctor’s certificate from the first day.

Your employer can often ask sooner than the law. Official guidance in Germany, Austria and Luxembourg says an employer may ask for a certificate from the first day.

Check your country in our doctor’s note checker. It shows whether you can self certify and when a certificate is due.

If your doctor is in another country, ask the body that pays your sick pay which format it accepts. Some want a specific national form. Others accept a signed certificate in a language they can read.

Do I need a certificate to go back to work?

In the UK, usually not. Official guidance says people do not need to be signed back to work, and the fit note has no option to do this. You can return whenever you feel able, even before the note ends.

Elsewhere, ask your employer. Some employers still ask for a document confirming you can return. Where they do, a return to work certificate usually records the date you can resume and any changes, such as lighter duties.

Our return to work certificate is planned, and you can join the waitlist. If your employer instead wants a statement that you are fit for a named job, a fitness certificate may fit. Ask your employer which one it means.

In Ireland, the Illness Benefit process works differently. Your doctor marks the last certificate of incapacity for work as final before you return.

What does a school or university usually want?

Each school, college and university decides which certificates it accepts. Read the student handbook or ask the student office first.

For a missed exam or deadline, a sick leave certificate is the usual evidence of absence from study. If the school accepts a note from you or a parent, our school absence letter generator helps you write it.

For a contagious illness, the school may have rules on when a child can come back. Our contagious period calculator explains typical periods.

School sport can follow national sports rules. In Moldova, a certificate for school physical education comes from a family doctor or a sports medicine specialist. In Croatia, school sport is handled by school doctors. Check the sports certificate rules for your country.

Do I need a medical certificate for a sports club or race?

It depends on your country, sport, level and age. Casual sport rarely needs paperwork. Club licences and official competitions often do.

In Italy, Croatia, Latvia, North Macedonia and Monaco, every level of organised sport needs an exam in person. In Portugal, Luxembourg, Hungary, Greece, Cyprus, Albania and Andorra, federation registration or licences need one.

Most big marathons we checked ask only for a declaration that you are fit. London, Dublin, Amsterdam, Vienna and Copenhagen are examples. Italian races ask foreign residents for a health form with Italian competitive tests.

If your club confirms in writing that a note from any doctor is enough, with no tests, our sports fitness certificate may fit. It is valid for 12 months, though your club may ask for a newer one. The sports certificate tool shows the rule for your country.

What does an airline or travel insurer ask for?

Airlines set their own rules. Many ask for medical clearance, often on their own form called a MEDIF, after recent surgery, a serious illness, a new cast or a contagious illness.

With a MEDIF, you fill in the first part and your doctor completes the rest. The airline’s medical team then decides. Our fit to fly checker shows typical waits and when airlines tend to ask.

In pregnancy, an airline may ask for a letter from your doctor or midwife after week 28. Some airlines want their own form instead of a letter. Use the pregnancy fit to fly calculator to check your weeks.

Travel insurers set their own evidence rules for claims. Ask your insurer which document it needs and whether it has its own form. Our travel insurance claim letter helps with the covering letter.

Our fit to fly certificate is planned. You can join the waitlist now.

Can a consulate accept an online medical certificate?

On the official pages we checked, no European destination yet confirms that it accepts a certificate from any licensed doctor. Most that ask for one name a panel doctor, a local clinic or tests.

Belgium, Spain, Greece, Hungary, Bosnia and Herzegovina and Andorra list a certificate before the visa for the visas or permits we checked. The Spanish consulates in London and Manchester accept only certificates issued in the UK or Spain.

So for a visa, the tool never offers our certificate today. It sends you to the visa medical certificate finder, which shows who can issue it and how recent it must be.

What about a driving licence medical?

Driving licence medicals follow the rules of the licensing authority. They are not something we offer.

In the UK, the medical for a lorry or bus licence goes on form D4, which a doctor must fill in. That can be your GP, though an optician might need to fill in the eyesight section, or a private firm that does drivers’ medical exams. For a car, you must be able to read a number plate from 20 metres away. Lorry and bus drivers have a stricter eyesight standard.

UK drivers must also tell the licensing agency if they develop a notifiable medical condition. If you exchange your licence for one from another EU country, that country’s rules on medical checks apply to you.

Check the driving licence medical requirements page for your country.

Worked examples

These cases use hypothetical people and the published rules behind each tool.

Sam, UK, employer, illness absence. Sam has been off for 9 days in a row. The tool says a sick leave certificate fits, because a fit note is needed after 7 days. It adds that a UK fit note assessment can be face to face, by phone or online. The result shows "Our certificate may fit" and links to the doctor's note checker.

Giulia, Italy, sports club, fitness. Giulia wants to join a running club. The tool checks the Italian sports rules. Every level of organised sport needs an exam in person, and the non competitive certificate includes an ECG. The result shows "Not an online certificate: book in person" with no price.

Priya, UK resident, consulate, visa. Priya is applying for a Spanish non lucrative visa through the London consulate. The tool routes her to the visa finder. London accepts only certificates issued in the UK or Spain, and a sworn translation into Spanish. The result shows what to book instead.

Tom, Germany, airline, travel. Tom has a cast fitted and a flight in two days. The tool sends him to the fit to fly checker, which lists airline rules on fresh casts. Our fit to fly certificate is planned, so the result shows the waitlist.

Ana, Portugal, someone else, gym. Ana's gym asks for "a medical certificate". The tool asks whether the gym named a form or test. Ana says no. The result shows "Check first": ask the gym whether a certificate from any doctor after an online consultation is fine. If yes, our fitness certificate may fit.

When to see a doctor straight away

A certificate is never more urgent than your health.

Call 112, the free emergency number across the EU, or your local emergency number if you have:

  • chest pain or pressure
  • severe difficulty breathing
  • sudden weakness, numbness, a drooping face or trouble speaking
  • a seizure, fainting or confusion
  • heavy bleeding that will not stop
  • thoughts of harming yourself

Contact a doctor soon if a fever lasts more than a few days or you keep getting worse. If the tool sees an urgent answer, it stops and shows this advice only.

How getmedicalcertificate.com can help

We offer sick leave, fitness, sports fitness and general medical certificates. Our sports fitness certificate is valid for 12 months. Fit to fly and return to work certificates are planned, and you can join the waitlist.

A doctor reviews your case and decides. You pay EUR 34.99 only after that review, with no added fees at checkout. If the doctor cannot issue a certificate, you get a full refund.

Approved certificates arrive as a signed PDF, usually within about 30 minutes of the doctor’s approval. Each carries a reference code anyone can check. We cover 42 countries in Europe, and certificates are issued in English.

Where a rule needs an exam in person, a named doctor or a test, the tool says so and does not offer our certificate.

Find your document above, then start your request if it fits.

Frequently asked questions

Start with who is asking and why. An employer checking a sickness absence usually wants a sick leave certificate. A sports club wants a sports fitness certificate, an airline a fit to fly letter, and a consulate a visa health certificate. If the person asking names a form, doctor, clinic or test, use exactly that.

A sick leave certificate says you were too unwell to work or study for set dates. It is evidence of an absence. A fitness certificate says you are fit for a named job, course or activity. It looks forward rather than back. Employers usually want the first for a sickness absence and may want the second before a new role.

No. An online doctor cannot do a physical exam, an ECG, a blood test or an X ray. Many competitive sports certificates, visa health certificates and driving licence medicals need one of these, or a named doctor or clinic. For those, book in person. For a sick leave, fitness or general certificate, ask the person asking whether they accept one after an online consultation.

Usually not. Official UK guidance says people do not need to be signed back to work, and the fit note has no option to do this. You can return whenever you feel able, even before the note ends. If your employer asks for its own return to work paperwork, check your workplace policy or ask HR.

It depends on the country, sport and level. In Italy, Croatia, Latvia, North Macedonia and Monaco, organised sport needs an exam in person. In countries with no national rule, such as Germany or Ireland, the club sets its own rule. If the club accepts a note from any doctor with no tests, a sports fitness certificate valid for 12 months may fit.

On the official pages we checked, no European destination yet confirms it accepts a certificate from any licensed doctor. Many name a panel doctor or local clinic, or need a chest X ray or blood tests. Some, such as France and the Netherlands, check your health after you arrive. Use the visa finder to see what your destination asks for.

Only if your airline asks for one. Airlines often ask for medical clearance after recent surgery, a serious illness, a new cast or a contagious illness, sometimes on their own MEDIF form. In pregnancy, some airlines ask for a letter after week 28. Check your airline's medical page, and use the fit to fly checker for typical waits.

The licensing authority decides. In the UK, the lorry or bus licence medical goes on form D4, filled in by a doctor. That can be your GP, with an optician possibly filling in the eyesight section, or a private firm for drivers' medicals. We do not offer driving licence medicals, so the tool sends you to the driving licence page.

Then follow that rule. Some countries send sick leave certificates through national systems, such as the electronic certificate in Germany or the secure official form in France. If your doctor is in another country, ask the body that pays your sick pay which format it accepts. Some want a national form; others accept a signed certificate they can read.

It means an online certificate may work, but the person asking has the final say. Before you order, ask them whether they accept a certificate from any registered doctor after an online consultation. If they say yes, our certificate may fit their request, though a doctor still decides. If they name a form, doctor or test, use that instead.

This finder gives general information, not legal or medical advice. The person who asked for the certificate decides what they accept, and a doctor decides whether to issue one. If you feel very unwell, contact a doctor or emergency services.
