Medical Certificate in the United Kingdom and Doctor's Note in the United Kingdom: What's the Difference?
In the UK, employees can self-certify sickness absence for the first 7 calendar days without any certificate. From day 8, a "fit note" (officially the Statement of Fitness for Work, form MED3) is required, issued by a GP, registered nurse, pharmacist, physiotherapist, or occupational therapist. For sport, gym, travel, or general purposes (insurance, visa, education), a separate certificate type applies. See the full list below. In everyday English this is often just called a doctor's note for the United Kingdom, even where the official local term differs.
UK Legal Snapshot: Sick Leave / Fit Note
| Local document name | Statement of Fitness for Work, commonly called a "fit note" (form MED3). Formerly known as a "sick note," renamed in 2010. |
|---|---|
| Self-certification period | First 7 calendar days of sickness absence; no certificate legally required |
| When a certificate is required | From day 8 of continuous sickness absence |
| Who can legally issue it | GP, registered nurse, pharmacist, physiotherapist, or occupational therapist (since July 2022) |
| Submission deadline | As soon as reasonably possible to your employer, per your employer's sickness policy |
| Governing framework | Statutory Sick Pay (SSP) rules and the Social Security (Medical Evidence) Regulations 2022 |
| Data protection law | UK GDPR and the Data Protection Act 2018 |
Note on Statutory Sick Pay: SSP rules changed from 6 April 2026: day-one entitlement applies, waiting days were abolished, and the Lower Earnings Limit was removed. Confirm the current weekly rate and maximum duration with gov.uk or Acas at the time you need it, as these figures are reviewed periodically.
A private fit note from a GMC-registered clinician is legally valid whether obtained online or in person. There is no blanket statutory duty for an employer to accept a private note during the first 7 days (the self-certification window), but from day 8 a validly issued fit note fulfils the legal requirement. Sources: gov.uk, NHS, Acas. Last reviewed July 2026. Always confirm current figures before relying on them for a specific case.
In practice, many UK employers accept a simple verbal or written self-certification for the first week. Some ask employees to complete an internal form modelled on the SC2, while others just want a message before their shift. Where a fit note is needed from day 8, it can cover a wide range of conditions, including stress, anxiety, and other mental health conditions, which are treated the same as physical illness under UK employment law. A fit note can state either that you are "not fit for work" or "may be fit for work" with specific adjustments (such as reduced hours, amended duties, or a phased return); our doctors will discuss which applies to your situation during the assessment rather than defaulting to the stricter option.
If an employer disputes a validly issued fit note, the correct route is not to withhold pay outright but to raise the concern with the employee and, if unresolved, seek guidance from Acas. A fit note issued by a GMC-registered clinician after a genuine assessment is not invalidated simply because the employer would have preferred a different outcome.
All Certificate Types Available in the UK
Every certificate type is reviewed by a GMC-registered doctor against UK requirements. Choose the one that matches your situation. Most UK requests fall into one of these categories, and if you're not sure which applies, our assessment will ask enough questions to route you to the right one before you pay anything.

Who Accepts These Certificates in the UK
A certificate is only useful if the institution asking for it will actually accept it. Below is who we most commonly hear from, and what each one is typically looking for when a certificate lands on their desk.
Employers
A validly issued fit note from a GMC-registered clinician satisfies UK Statement of Fitness for Work requirements from day 8, and HR teams can independently verify the doctor's registration number and the certificate's reference number.
Schools & universities
Accepted for mitigating circumstances claims, exam deferrals, and school sports medical clearance, including for contact sports that require a documented health check before participation.
Gyms & sports clubs
Accepted for gym membership sign-up, personal training onboarding, and club/league medical clearance requirements, particularly for higher-risk or contact activities.
Airlines
Fit-to-fly certificates accepted by UK airlines requiring medical clearance for travel after surgery, illness, or later-stage pregnancy, ahead of departure from a UK airport.
Government & insurance

UK Pricing: All Certificate Types

|---|---|---|
| Sick Leave Certificate | €34.99 | €50-€120 |
| Sports Fitness Certificate | €34.99 | €50-€120 |
| General Fitness Certificate | €34.99 | €50-€120 |
| Fit to Fly Certificate | €34.99 | €50-€120 |
| Return to Work Certificate | €34.99 | €50-€120 |
| General Medical Certificate | €34.99 | €50-€120 |
Private GP visit costs are typical ranges reported by UK private clinics and vary by location and provider.
The gap between our pricing and a typical private GP visit reflects the difference in overhead between a walk-in clinic appointment and an online assessment. You're paying for the doctor's review and the certificate itself, not a consultation room, receptionist time, or travel. NHS GP appointments are free at the point of use but are not designed to issue same-day private documentation for insurance, travel, or sport, which is why most people in this position turn to a private route regardless of price.

The UK online sick note market is crowded, but most services compete on a single product at a single price point. The table below reflects typical positioning across that market as of July 2026. Individual competitors vary, so treat this as a general comparison rather than a claim about any one named provider.

Our UK Medical Team
Every UK certificate is reviewed by a doctor registered with the General Medical Council (GMC). Registration numbers are shown on your certificate and can be independently verified on the GMC website. There is no need to take our word for a doctor's credentials when the public register confirms them directly.
A clinician will only issue a certificate after genuinely assessing your case, based on the health information you provide and, where needed, follow-up questions. If a certificate is not clinically appropriate, for example if your case needs an in-person examination or further investigation, the doctor will decline to issue one and you receive a full, automatic refund. We do not operate on a pay-first, questions-later basis: no certificate is guaranteed simply because a request was submitted.
If you ever have a concern about the assessment you received, our complaints process sits alongside, not instead of, your right to raise a concern about a specific doctor directly with the GMC. We would always prefer to resolve a concern ourselves first, but we will never suggest that a GMC-registered clinician's public accountability is somehow replaced by using our service.
Registered practitioners, not a call centre.
Every doctor is independently verified against their national medical council before they see a single patient - including across United Kingdom.

Yes. A private fit note issued by a GMC-registered clinician after a genuine online assessment is legally valid in the UK, the same as one issued in person. You do not need to attend a surgery in person from day 8 of sickness absence.
No. UK employees can self-certify sickness absence for the first 7 calendar days without any certificate. A fit note is only required from day 8 onward.
A fit note issued by a GMC-registered clinician meets the legal Statement of Fitness for Work requirement from day 8. Employers cannot dismiss a validly issued fit note simply because it was obtained online rather than in person.
Since July 2022, a fit note can be issued by a GP, registered nurse, pharmacist, physiotherapist, or occupational therapist, in addition to a hospital doctor. Digital issue without a wet-ink signature is valid.
Our UK sick leave certificate costs €34.99, which includes the doctor's review, your PDF certificate with certificate verification, and same-day email delivery. This compares to €60-€120 typically charged for a private GP appointment.
Many UK schools, football and rugby clubs, and martial arts gradings require a sports fitness certificate confirming medical clearance to participate, particularly for contact sports. Our sports fitness certificate is valid for 12 months.
Yes. Airlines departing UK airports may require medical clearance to fly after recent surgery, illness, or during later pregnancy. Our fit-to-fly certificate is reviewed against your specific procedure and travel date.
Yes. We are compliant with UK GDPR and the Data Protection Act 2018. Your medical information is only seen by the doctor assessing your case and is never shared with third parties.
A doctor can only certify a period based on a genuine clinical assessment of your condition, which may include days immediately before your assessment if that is clinically supported. We do not issue certificates backdated to cover a period simply because it was requested, as this would not reflect a genuine medical assessment.
Self-employed workers are not entitled to Statutory Sick Pay and do not need to submit a fit note to an employer, but a medical certificate can still support an insurance claim, a client-facing absence explanation, or an application for Employment and Support Allowance where relevant.
Yes. In everyday English, "medical certificate" and "doctor's note" describe the same kind of document, and we use both terms for the United Kingdom. Whichever term the person or institution asking for one uses, our certificate is reviewed by a doctor and designed to meet that request.
