Sjúkrahúsið á Akureyri is the second-largest hospital in Iceland and, with Landspítali in Reykjavík, one of only two specialist hospitals in the country, running an emergency department, intensive care, a maternity ward, a paediatric clinic and a range of specialist departments. That makes the city the medical centre for the whole of northern and eastern Iceland — which is a very different thing from making it an easy place to obtain a short administrative document.
The reason is capacity rather than distance. A hospital and health centre serving a region that size prioritise by clinical need, and documenting a minor self-limiting illness is properly at the bottom of that order. An emergency department in particular is the wrong place to seek a certificate, because doing so consumes a slot someone else needs.
