Getting a Medical Certificate in Milan

Milan is Lombardy's capital and Italy's financial, fashion and business centre, with a dense concentration of corporate headquarters, financial firms and design and fashion houses. A city built around business travel and fast-moving schedules is also a city where the practical obstacle to getting documentation quickly is very often the appointment itself, rather than the underlying need.

The city carries a large student population too, between Politecnico di Milano and Università degli Studi di Milano among others. Illness affecting an exam, a thesis deadline or coursework is generally handled through a formal process set by the individual faculty, so it is worth reading what yours actually requires before requesting anything.

There is no clinic to visit in Milan: your case is assessed online by a doctor registered to assess cases in Italy, and the review happens the same way wherever in the country you are. You do not need to be well enough to travel across Milan or to get a same-day appointment for this to work.

Milan's real constraint is primary care, and Lombardy's numbers are the most extreme in Italy. GIMBE's analysis put the region short of 1,540 medici di medicina generale at 1 January 2025 — by a distance the largest regional gap in the country, more than double second-placed Veneto — with an average of 1,533 patients per family doctor against a national average of 1,383 and a contractual ceiling of 1,500. That is not a story about Milan lacking medicine. The city holds Niguarda, with more than 1,300 beds and around 4,500 staff, San Raffaele at roughly 1,350 beds, the Policlinico di Milano at around 900 and Humanitas at Rozzano. It is a story about the ten minutes of a full list that assessing and signing a certificate requires — which is exactly the task a hospital is not for.

The other Milanese pattern is travel. The Milan airport system carried 56.9 million passengers in 2024, the largest in Italy, with 28.5 million through Malpensa and 10.6 million through Linate, and the city's corporate and fashion employers generate a matching volume of short-notice international travel. Fit-to-fly documentation is the certificate type this produces: someone already booked onto a flight who has had recent surgery, is in later pregnancy, or has a condition an airline wants confirmation about. Because carriers set their own gestational limits and post-operative intervals, and several require the certificate dated within a few days of departure, the useful sequence is to find the airline's policy first and have the case assessed against it. Corporate HR in Milan is also among the most process-driven in the country, which is why return-to-work documentation is asked for more formally here than in many Italian cities.

Italy's business and finance centre

Corporate, financial and fashion employers concentrate here, and business travel schedules are tight. Getting documentation quickly without a same-day appointment is a common practical need.

Politecnico di Milano and Università degli Studi di Milano

Both, alongside Milan's other institutions, set their own evidence requirements for illness affecting exams or coursework. Check your own faculty's process first.

A major travel hub

Fit-to-fly documentation for travel through Malpensa or Linate after surgery, illness or during later pregnancy. Airline requirements vary, so confirm with the airline directly too.

For statutory Italian sick pay, start with your own doctor

Our certificate is a separate general medical document. If you need the INPS-linked electronic sickness certificate for employer sick pay, that comes from your medico di base or, out of hours, the guardia medica — see the national rules above.

All Certificate Types Available in Italy

Every certificate type is reviewed by a doctor registered to assess cases in Italy. Choose the one that matches your situation, and see the legal snapshot above if what you actually need is Italy's formal INPS sick-pay certificate rather than a general medical certificate.

How It Works in Milan

  1. Complete assessment

3-5 minutes. Select your certificate type, confirm Italy as your country, and answer relevant health questions honestly and in as much detail as you can. No payment is taken at this stage.

  2. Doctor registered to assess cases in Italy reviews

Your case is reviewed against the context of your situation in Italy, generally during business hours (CET/CEST, Monday-Friday). The doctor may request more information before deciding whether it is clinically appropriate to issue your certificate.

  3. Receive your PDF

Emailed same day, usually within 30 minutes of approval, with doctor credentials and a verification reference that lets a school, club, employer, or other recipient confirm authenticity.

What a Milan Medical Certificate Is Used For

General medical documentation

Supporting documentation for everyday situations where evidence of a medical assessment is useful, issued without an in-person appointment.

Insurance and legal matters

Insurers and legal processes commonly ask for evidence that a genuine medical assessment took place. Check what your specific insurer or process requires — the national rules above cover the general position.

Universities and coursework

Milan's universities each run their own process for illness affecting exams or coursework. This can support that process, but the faculty's own evidence requirements are what matters — check them first.

Not a substitute for statutory sick pay documentation

If you need paid sick leave recognised by an employer through INPS, that requires the certificato di malattia telematico from your medico di base or the guardia medica — not this certificate. See the national rules above.

Our Medical Team for Italy

Every certificate for Italy is reviewed by a doctor registered to assess cases in Italy. A clinician will only issue a certificate after genuinely assessing your case, and only if it is clinically appropriate to do so. If the doctor cannot issue your certificate, you receive a full refund rather than a certificate you cannot use.

Registered practitioners, not a call centre.

Every doctor is independently verified against their national medical council before they see a single patient - including for patients in Milan.

Medical Certificates in Milan: Common Questions

No. The assessment is completed online and reviewed by a doctor registered to assess cases in Italy, so where you are in the country makes no difference to the process or the price. This page exists because people search for their own city, not because the service works differently in Milan.

No. The INPS-linked electronic sickness certificate that Italian employers rely on for statutory sick pay is generally issued by your own medico di base, or out of hours by the guardia medica (continuità assistenziale), and transmitted directly to INPS. Our certificate is a separate, complementary general medical document — for insurance, visa, education, sport or travel purposes. If you need paid sick leave through INPS, contact your medico di base or the guardia medica.

Our certificate is €34.99 flat, including the doctor's review, the PDF and its verification reference. A private doctor visit in Italy commonly falls in the €50-€120 range depending on the type of consultation. A certificate is not a substitute for care, though — if you need to be examined or treated, see a doctor.

It can be useful supporting general medical documentation, but it is not the certificato di malattia telematico that Italian statutory sick pay is built on. For anything tied to employer sick pay through INPS, see the national rules above and contact your medico di base or the guardia medica instead.

Same day, usually within 30 minutes of the doctor approving your case during business hours. Some cases take longer where the doctor needs more information from you before making a decision.

Ours is €34.99 flat. There is a free route for one specific thing: the INPS certificato di malattia for statutory sick pay is issued at no charge by your medico di base, or by the guardia medica out of hours, and if that is what you need you should not be paying anyone for it. For the other five types — sports fitness, general fitness, fit to fly, return to work, general medical — there is no free public equivalent, and a private appointment in Milan for the same purpose generally costs several times more. The price is identical across types, so the decision is about which document you need rather than which you can afford.

Not the Italian one, and this is the clearest refusal in our range. Competitive entry — a race under a CONI-recognised federation, or membership of a club registering you as an agonista — requires the certificato di idoneità agonistica, which only a sports-medicine specialist can issue and which involves an ECG at rest and after exertion, plus spirometry for athletics. The non-competitive certificate is easier but still needs an in-person visit with a physical examination, blood pressure check and a properly reported resting ECG. Where our sports fitness and general fitness certificates do apply is the gym and studio layer: recreational activity, which has not legally required a certificate in Italy since 2013 but which operators keep asking for anyway.

The doctor reviewing your case — and the certificate itself says considerably less than your submission does. A certificate is designed to be handed to an employer, a school or an airline, so it states that an assessment took place, what it concluded and the dates it covers, not your full history. That distinction matters in a city where HR departments are large and documents circulate: what you tell the doctor is clinical information, and what goes to your employer is a conclusion. If a Milan employer is asking for your diagnosis rather than a fitness conclusion, that is worth questioning before you supply it.

Usually, and the limiting factor is the airline rather than the assessment. Approved certificates are sent the same day, typically within about 30 minutes of the doctor signing off during business hours, so two days is comfortable. What is not comfortable is discovering at 48 hours that your carrier wants its own form completed by an examining doctor, a certificate dated inside a narrow window before departure, or a named specialist's letter. Pull up the operating airline's medical policy now and tell us exactly what it asks for, and the case is reviewed against that. With 56.9 million passengers through Milan's airports in 2024 the carriers all have well-documented policies — they are simply not the same as each other.

Asking for the wrong type, by a distance. The two most often confused are sick leave documentation and the INPS certificato di malattia: people buy a certificate stating they were unwell and then discover HR needed a telematic filing only a medico di base can make. Second most common is a return-to-work step being answered with an absence certificate, which does not say the thing a re-entry process is asking about. Get the requirement in writing, name the document, and the type follows. Where the requirement is an occupational-health fitness judgement, that belongs to the company's medico competente and no external certificate substitutes for it.
