No. It is a layout mock-up, not a medical document. No doctor has examined anyone or signed anything, so it certifies nothing and carries no legal or medical weight. Every file the tool produces is watermarked to make that unmistakable.
No, and there is no paid tier that removes it. The watermark is drawn into the document itself, so it survives the PNG, SVG and PDF exports. A clean copy would be a blank anyone could forge, which is not what this tool is for.
Seeing what a certificate contains before you need one, checking which details an employer will expect to find on it, mocking up a layout for a practice template, and teaching or training material. It answers "what does one look like" without pretending to be one.

The generator asks for an email address and phone number before it will export, but only so the sample is filled in like a real form would be — they stay in your browser with everything else on this page, are never sent to us, and do not create an account or subscribe you to anything.

The patient's name, the date of examination, what the doctor is certifying and for what period, any advice or workplace adjustments, and the doctor's name, signature and registration number. The generator lays out those same fields so you can see where each one sits.
A registered doctor has to assess you and issue it. You can do that online here: complete a health questionnaire, a doctor reviews your case and comes back to you with any questions, and if they are satisfied they issue a signed PDF the same day — a flat €34.99, refunded in full if no certificate is issued.
