If your role carries a statutory occupational-health requirement, that clearance comes from a company doctor in person. And after a long absence, larger Munich employers typically run their own structured reintegration process with their occupational health service — that process is theirs, and a certificate doesn't replace it.

Your own practice is the only route that can offer a telephone sick note, because that requires the practice to know you already, and it is the only one that gives you continuity if symptoms drag on. The problem is that commuting is exactly what makes it hard: with a median commute of 41 km, a practice near work is convenient when you are well and useless when you are not, and one near home often only has slots during the hours you are on a train. An online assessment removes the geography from the documentation part; it doesn't replace having a Hausarzt.
A doctor can assess and decide remotely, and the assessment here is a real one — cases are declined where the information doesn't support a certificate. What remote assessment cannot do is produce findings that need hands-on examination or a test: a resting ECG, a wound check, a lung function measurement.
