Medical Certificates in Belgrade: Who Needs Them?

Belgrade is where most of Serbia's office economy sits, and the pattern of certificate requests reflects that. Serbia's ICT sector employs close to 126,000 people — up around 39% since 2020 — and a large share of those jobs are in New Belgrade's business towers, working for German, Swiss and US clients on contracts that require written documentation of any absence. Add the banking, telecoms and shared-services employers clustered around the same district and you get a steady stream of people who need a doctor's note in English, not just a Serbian-language form.

The public healthcare route in Belgrade works, but it is slow at the front door. The University Clinical Centre of Serbia is one of Europe's largest hospital complexes, with over 3,000 beds and roughly 25,000 patient contacts a day, and the dom zdravlja network that feeds it is under real pressure: the Physicians' and Pharmacists' Union of Serbia estimates around 6,000 doctors have left the country in the past decade, at an average of roughly 600 a year. Professional associations put the national shortfall at 5,000-plus healthcare workers, including at least 1,000 doctors. That is felt most acutely as same-day primary-care slots that simply are not there.

Commuting makes it worse. The Soko high-speed service now links Novi Sad to Belgrade in 36 minutes, and New Belgrade railway station is used daily by commuters from Novi Sad, Inđija and Stara Pazova, which means a large part of the capital's workforce does not live near the health centre where they are registered. If you are registered with an izabrani lekar in Stara Pazova but you work on Milutina Milankovića, getting an in-person appointment can cost you a whole working day.

Travel documentation is the other big driver. Belgrade Nikola Tesla Airport closed 2025 with a record 8.91 million passengers — up 6.5% year on year, with more than a million passengers in August alone and 116 international destinations served in the summer season. Airlines, tour operators and travel insurers routinely ask for written confirmation that a passenger is fit to fly after a recent illness, injury, surgery or in later pregnancy, and that request almost always arrives with a deadline measured in hours rather than weeks.

The fifth group is students and athletes. The University of Belgrade enrolled 94,425 students in 2024-25, and faculties ask for medical documentation to support deferred exams and absence from practicals. Amateur sport is just as demanding: race organisers, gyms, martial arts clubs and football academies across the city ask for a general or sports fitness assessment before sign-up. None of those uses touch the RFZO system at all, which is where an independent online certificate fits most cleanly.

First check what they actually need. If they need a statutory bolovanje for paid sick leave, no private certificate — ours or anyone else's — will do the job, and you need your izabrani lekar. If it is an internal absence record, an unpaid day, a return-to-work sign-off or a discretionary policy, an independent doctor's report is normally exactly what is wanted, and it helps to send it in writing with the doctor's registration details visible.

Often yes. Our Fit to Fly assessment looks at your recovery, the flight duration and any airline-specific timeframes, and the doctor either confirms fitness to fly or explains why not. If your surgery was very recent or your airline applies a fixed post-operative waiting period, the honest answer may be that you need to wait or get clearance from your treating surgeon.
