Under the 2022 Sports Act the Consejo Superior de Deportes is progressively introducing a compulsory pre-licence medical in the sports where it is judged necessary, with the requirement delegated to the comunidades and then the federations — so it varies by federation and sometimes uses their own form. Validity is not a fixed number: it is set by what the certificate states and by what the institution reading it will accept, and many organisations treat medical documentation as stale after a few weeks. On backdating, the honest answer is that a doctor certifies what they have assessed, so a certificate cannot simply be dated to cover a period before the assessment. A doctor may, where clinically reasonable, describe a period of illness you report — but that is a clinical judgement they make, not a field you fill in, and any service that offers to backdate on request is not one to trust with a medical document.

If your faculty needs an absence from teaching or an assessment justified, that runs through its own procedure with its own evidence list, and general medical documentation of the illness is usually what fits.
