Read your staff regulations or ask HR what evidence they expect, then name that body and its requirement in your submission so the wording lines up.
It's the right type for that request, and its independence is structural — the assessing doctor is not your treating physician, which is usually precisely what an insurer means by independent confirmation. Whether it satisfies a particular insurer depends on what they specified: some want a named form completed, some want records from the doctor treating you, some want a formal medical expert report, which is a different exercise entirely. Put the insurer's own wording into your submission so the document speaks to what was asked. If they've requested something only a treating doctor or an appointed expert can provide, we'll say so rather than issue something they'll reject.
