Medical Certificates in Kyiv: Who Needs Them?

Kyiv is not part of Kyiv Oblast. It is one of only two cities in Ukraine with special status under Chapter IX of the Constitution, which makes it a first-level administrative division in its own right, subordinated directly to national government while also hosting the administrative bodies of the surrounding oblast. That constitutional quirk matters more than it sounds for paperwork, because the national institutions that govern sick leave — the Ministry of Health, the National Health Service of Ukraine and the Pension Fund of Ukraine — all sit inside the city. Kyiv residents are therefore closer to the machinery of the statutory system than anyone else in the country, and no closer at all to a same-day appointment.

The statutory route in Ukraine is entirely electronic and entirely inside one system. A doctor creates a medical conclusion on temporary incapacity for work in the eHealth system, known in Ukraine as ЕСОЗ; that conclusion generates an electronic sick-leave record in the Pension Fund register; and the money follows, with the first five calendar days paid by the employer and the sixth day onward financed by the Pension Fund. The right to create that conclusion depends on whether the doctor satisfies Ukrainian legal requirements to practise, not on whether the clinic is state-run or private. Our doctors are registered with the General Medical Council in the UK and do not hold accounts in ЕСОЗ, so we cannot open, close or transmit a лікарняний, and we do not pretend otherwise.

What we do issue is a separate document: a written record of a registered doctor's assessment of your case, with a verification reference on it. That is the thing a great many requests actually need. An employer asking for evidence behind a short absence, a faculty office assessing a deferral request, an airline applying a fit-to-fly policy, a gym or federation requiring a fitness declaration, an insurer or a visa service asking for a medical statement — none of those sit inside ЕСОЗ, and none of them need a лікарняний. Confusing the two is the single most common reason people buy the wrong document, so it is worth reading the request carefully before you pay for anything.

Kyiv's student population is one of the largest concentrations of certificate requests in the country, and it is spread across institutions founded in three different centuries. The National University of Kyiv-Mohyla Academy traces its foundation to 1615, when the noblewoman Halshka Hulevychivna endowed the school that became it. Taras Shevchenko National University of Kyiv opened in 1834 as the Imperial University of St Volodymyr and took its first 62 students in July that year. Igor Sikorsky Kyiv Polytechnic Institute was founded on 31 August 1898, funded substantially by an association of sugar-mill owners. A student needing to document a missed examination is dealing with a faculty procedure, not a social-insurance one.

A lot of work that is nominally Kyiv-based is not physically done in Kyiv. Ukrainians in services, IT, administration and education are frequently working from a different city than the one on their registered address, or from outside the country, and the practical consequence is that the clinic nearest their registered residence is not the clinic they can walk into. Where the obstacle is distance, travel or a diary rather than a need to be examined, a remote assessment is a reasonable way to get documentation without pretending a journey is easy. Where you genuinely need looking at, a doctor should tell you to be seen in person — and ours will.

No, and you should be wary of anyone who says they can. A Ukrainian e-sick-leave record is generated from a medical conclusion created inside the eHealth system, ЕСОЗ, and only a doctor who satisfies Ukrainian requirements to practise can create that conclusion. Our doctors are GMC-registered in the UK and work outside ЕСОЗ. What we issue is a separate certificate documenting a doctor's assessment, which is what many employers, faculties and insurers actually ask for.

Under the Ukrainian system the employer pays the first five calendar days of temporary incapacity, and from the sixth day the Pension Fund of Ukraine finances the payment. Buying a certificate from us does not start, alter or accelerate any part of that. The payment chain runs off the electronic record in the Pension Fund register, which our certificate is not.

It is the same €34.99 for all six types we issue: Sick Leave, Sports Fitness, General Fitness, Fit to Fly, Return to Work and General Medical. There is no surcharge for a particular type and no separate doctor's fee added afterwards. What differs between them is the information the doctor needs from you, not the price.

Not for the assessment. The doctor is assessing your described condition rather than your address, and a great many Ukrainians are currently working from somewhere other than their registered residence. What can matter is the institution's own rule: some employers and faculties specify where documentation must come from, so check the policy wording before you apply rather than after.

Usually yes, because a deferral is an academic decision governed by the faculty's own procedure and not by the social-insurance system. Read the procedure first: some faculties require the request within a fixed number of days, some want it lodged through a dean's office rather than emailed to a lecturer, and a perfectly good certificate submitted the wrong way is still refused.

Cases are reviewed while a doctor is on, and approved ones are issued the same day, typically within about half an hour of the decision. The usual delay is not the review but a question waiting for an answer, so being specific first time — when symptoms began, what they stop you doing, what your work or study actually involves — is the single biggest thing that keeps it same-day.

For some conditions, yes, and for others no. The judgement belongs to the doctor, who is professionally accountable for deciding whether your case can be assessed safely on the information you give, and who declines rather than issues where it cannot. That decision is the substance of the review. If your symptoms need looking at, you will be told to be seen in person, and you should go.

Every approved certificate carries a verification reference so that whoever receives it can confirm it came from the doctor who reviewed the case rather than out of a word processor. That reference is worth considerably more than a signature, which is easier to fake than to check. If HR asks how to verify it, point them at the reference printed on the document.

If you need the statutory record, yes — that has to come from a doctor working inside ЕСОЗ, and our certificate does not substitute for it. You should also see a Ukrainian doctor if your symptoms continue beyond the period we have documented, worsen, or need treatment rather than documentation. Then no certificate is issued, and that is the assessment working as intended rather than failing. A doctor who issues on request regardless of what is described is not performing an assessment at all, and a document produced that way is worth nothing to the employer or faculty reading it. Where a case is declined you are told why, and if the reason is that you need examining, that is the advice to act on.
