Medical Certificates in Odesa: Who Needs Them?

Odesa was founded in 1794 by decree of Catherine II, built as a warm-water port on the site of the Khadzhibei fortress, and the classicist grid that went up over the following century — wide perpendicular tree-lined streets, two- to four-storey buildings — is the reason UNESCO inscribed the Historic Centre of Odesa on the World Heritage List in January 2023. Institutionally the city grew from that port. Odesa National University was created in 1865 by edict of Alexander II, reorganising the Richelieu Lyceum into a university, and the medical school that became Odesa National Medical University began in 1900 as its medical faculty before becoming an independent institute in 1922.

That maritime history creates one distinction that matters more in Odesa than almost anywhere else in Ukraine, and we want to be blunt about it. A seafarer's medical fitness certificate is a regulated document under international maritime rules, and it must be issued by a medical practitioner specifically approved for the purpose. We are not an approved maritime practitioner, so if you are joining a vessel, renewing a seafarer's book or satisfying a manning agent, we are not the right service and you should go to an approved centre. Nothing we issue substitutes for that certificate, and no amount of wording on a document changes what a port state inspection will accept.

The second distinction is the statutory one that applies everywhere in the country. Ukrainian sick leave runs on the electronic лікарняний: a doctor creates a medical conclusion on temporary incapacity for work in the eHealth system, ЕСОЗ; that generates an e-sick-leave entry in the Pension Fund of Ukraine's register; and the employer pays the first five calendar days before the Pension Fund finances the rest from day six. The right to create that conclusion depends on the doctor meeting Ukrainian requirements to practise. Our doctors are GMC-registered in the UK, work outside ЕСОЗ, and therefore cannot open, close or transmit a лікарняний or set any statutory payment in motion.

What is left after those two exclusions is still a large and ordinary set of needs. Odesa's universities run their own deferral procedures — the national university founded in 1865, the medical university whose faculty dates to 1900, the polytechnic, the food-technologies academy, the law academy — and none of them route an academic decision through social insurance. Private employers in trade, logistics, hospitality and services ask for evidence behind short absences under their own policies. Airlines apply fit-to-fly rules, insurers ask for medical statements, and clubs ask for fitness declarations, none of which involve the Pension Fund register in any way. Those are the requests our certificate is built for.

Access is the practical reason people use a remote assessment here rather than anything more dramatic. A same-day family-doctor slot for something minor competes with patients who need treating, and many people with an Odesa employment contract or an Odesa registration are currently living and working somewhere else entirely, which puts the clinic attached to their registration out of reach. Where the barrier is getting to an appointment rather than needing to be examined, an online review is a reasonable answer. Where you need to be examined, the doctor should say so and send you to be seen.

When you request it, say what activity or role the fitness statement is for, because the doctor is judging suitability for something specific rather than issuing a blanket declaration.

It is a common situation, and it is precisely why the clinic attached to a registration is often not usable. The variable is the receiving institution: if an employer or faculty specifies where documentation must originate, that rule stands regardless of where you are, so read it before you apply.

Sometimes, and it depends entirely on what you describe and what the doctor concludes. The period is the doctor's judgement, not your selection, and a request for a specific number of days does not oblige anyone to certify them. Describe the course of the illness honestly and let the assessment set the period; a certificate stretched beyond what is defensible is worth nothing to whoever reads it.
