Medical Certificates in Herceg Novi: Who Needs Them?

Herceg Novi sits at the mouth of the Bay of Kotor, and it is unusual among Montenegrin coastal towns in having health itself as a core industry. The Institute for Physical Medicine, Rehabilitation and Rheumatology "Dr Simo Milošević" in Igalo, founded in 1949 and majority state-owned, is described as the largest and most important multidisciplinary health and recreational centre in Montenegro and the wider region, with a staff of over 300 medical specialists, physiotherapists and nurses. It is also one of the teaching bases used by the University of Montenegro Faculty of Medicine. A town with a medical rehabilitation industry has an unusually high share of health-sector employment — and an unusually well-informed population when it comes to medical documentation.

The other half of the local economy is seasonal tourism, and the season is intense. Montenegro received around 3.03 million international visitors in 2025, and the coastal municipalities carry the bulk of that: Herceg Novi is named alongside Tivat, Kotor, Bar, Budva and Ulcinj as the areas with the highest recorded demand for seasonal workers. From May to October the town's working population swells with hospitality, spa, guiding and maritime staff on short contracts, most of whom have no settled relationship with a local doctor.

Montenegro changed how that seasonal workforce is employed. A new mechanism for the employment of seasonal workers was introduced from 1 May 2025, with the state paying contributions and subsidies, and the government has been preparing legislation for permanent seasonal worker status to reduce unemployment and ease the tourism labour shortage. Formalisation increases documentation requirements rather than reducing them: a worker who is properly registered is a worker whose absences are recorded.

Health service capacity does not expand with the visitor numbers. Local care runs through the town's health centre and the Igalo institute, with referral to Kotor and Podgorica for anything more complex, against a national backdrop of physician and nurse numbers well below EU averages, a documented shortage of specialist doctors and continuing health-worker emigration. In July and August the practical result is that a routine appointment intended to produce a document is competing with a population that has temporarily doubled.

There is one more local factor: the Croatian border is minutes away at Debeli Brijeg. A large number of Herceg Novi residents work, shop, receive treatment or hold family ties across it, and the town is a natural stop for traffic heading to and from Dubrovnik. Cross-border employers, insurers and agencies want documentation in a language they read from a doctor they can verify, which makes an English-language certificate more useful here than a local-language form.

It does not satisfy the rule itself. Montenegrin law requires you to deliver a doctor's certificate of temporary work inability to your employer within three days, and that document comes from a doctor inside the national system — the employer then pays 70% for the first 60 days before the Fond za zdravstveno osiguranje Crne Gore takes over. We are not part of that system and cannot file into it, so our certificate is independent supporting documentation and does not discharge that obligation.

Yes, and seasonal staff are among the clearest beneficiaries: nothing in our process requires local registration, insurance or a family doctor. The unavoidable trade-off is that it produces no statutory sick pay, so it documents your situation rather than paying you.

But employment in Croatia may require a Croatian occupational fitness examination or a specific national form, and our certificate substitutes for neither — confirm what they need first.

For general recreational water activity, often yes. Recreational scuba diving is different: dive operators and certifying agencies typically require a specific diving medical questionnaire and, where anything is flagged, an in-person examination by a doctor trained in diving medicine. We will not substitute for a dive medical.

Since the new mechanism took effect on 1 May 2025 with state-paid contributions and subsidies, more seasonal workers are formally registered, and a formally registered worker is one whose absences are recorded and need evidencing. That makes credible written documentation more important, not less.

The certificate documents the doctor's assessment of your fitness or incapacity, and you can present the same PDF to more than one employer. If the two roles have genuinely different physical demands, tell the doctor about both, because limitations relevant to one may not apply to the other.
