Medical Certificates in Kotor: Who Needs Them?

Kotor lives off the sea in two distinct ways, and both generate medical paperwork. The first is cruise traffic: the Port of Kotor opened its 2025 season on 10 April with MSC Opera, and expected around 500 cruise ship calls and roughly 700,000 cruise passengers over the year, with up to four ships alongside on a busy summer day, berthing directly in front of the UNESCO-listed old town walls. Handling that volume takes a large seasonal workforce in guiding, transfers, retail, hospitality and port services, most of it on short contracts.

The second is seafaring as a career. Kotor is home to the University of Montenegro's Faculty of Maritime Studies, established in 1959 as a Maritime College and made a faculty in 1981, with 159 students enrolled in 2024 on programmes aligned to international standards. Montenegro has roughly 15,500 registered seafarers, of whom an estimated 6,500 to 7,000 are active — a substantial proportion of the working population of the Bay of Kotor. Seafarers deal with a documentation regime of their own, which is precisely where being clear about what a certificate is and is not becomes essential.

Around 0.9 million visitors came to Kotor in 2025, up about 4%, and the municipality is listed among those with the highest recorded demand for seasonal workers, alongside Herceg Novi, Tivat, Bar, Budva and Ulcinj. Montenegro's new seasonal worker employment mechanism, in force from 1 May 2025 with state-paid contributions and subsidies, is formalising that workforce — and a formally employed worker is one whose absence needs documenting. For a guide or transfer driver hired for six months, that means finding written medical evidence without ever having registered with a doctor in the bay.

Health provision has to serve a permanent population plus a summer multiple of it. Local care is primary and emergency, with referral to Podgorica for anything substantial, against national figures showing physician and nurse numbers well below EU averages and a documented shortage of specialist doctors with an unfavourable age structure. In July and August the practical position is that non-urgent appointment capacity in the bay is effectively gone.

Kotor's geography adds a specific friction. The bay road is the only way in and out for much of the municipality, and in peak season traffic through Kotor and around the bay can turn a short trip into an hour or more. For someone who needs a document rather than treatment, the combination of a scarce appointment and an unreliable journey makes a remote assessment the sensible default rather than a fallback.
